Behavioral Health Home

The Behavioral Health Home (BHH) is a healthcare service delivery model focused on combining primary care, mental health services, as well as social services and supports for adults and children diagnosed with mental illness. The BHH is a benefit for Medicaid, Healthy Michigan Plan, and MIChild enrollees.

Participation is voluntary and enrolled beneficiaries may opt-out at any time. The BHH receives reimbursement for providing the following federally mandated core services: 

  • Comprehensive Care Management 
  • Care Coordination 
  • Health Promotion 
  • Comprehensive Transitional Care 
  • Individual and Family Support 
  • Referral to Community and Social Services
