Temporary FOIA and Due Process Request Procedure



With the recent situation regarding the spread of the novel coronavirus (COVID-19), we are implementing preventative measures to control  the spread of disease. We are requiring all persons to submit any Requests for Local Appeals or any Freedom of Information Act (FOIA) Requests via the OCHN website. 



  • Go to www.oaklandchn.org

  • On the left side of the page, click on “Recipient Rights”

  • In the center of the Recipient Rights and Advocacy page, click on the “Forms/Mental Health Code” button

  • On the Forms/Mental Health Code page, click on “Local Appeal Request Form”

  • Answer the questions on the form.  Once you are finished, the form will be automatically submitted, and you will receive an email verifying this.
